From time to time children may be photographed or electronically recorded while engaging in ministry activities or while attending ministry-sponsored activities.  I agree, without compensation, to permit Trinity Lutheran Church, its agents, contractor assigns, or employees to use, reproduce, exhibit, display, broadcast, distribute, and create derivative works using my child's photographic image or likeness and/or images of my child's work product in, including, but not limited to, the church’s publications, promotional materials, websites, alumni materials, and recordings for the purposes of promoting, publicizing, illustrating, or explaining the church, its activities or programs and for the use and benefit of the church for other purposes.  
     This permission includes print, broadcast, taped, DVD, CD, and all present and future forms of electronic and on-line media.
     No names of participants or other identifying, specific information will appear unless approved by church administrators.  Web-hosted photographs will only include first names or initials and identifying, specific information when they are used on password-protected web pages (with user access assigned by the church) or in church publications directed to church families.
