Note:  This club will be offered in Session II and Session III.  Students will enroll in each session separately.

Club Sponsor:  Ms. Gergen and Mrs. McDermott

Grade(s):  5th-6th
Meeting Day/Time:  Monday, 3:00p.m. - 4:00p.m.

Club Overview:
Students will engage in games, lessons, and crafts together.

A parent/guardian should complete this form.  The application serves as a permission form for your child to participate in the club.  

Depending on the number of students who apply, students may be randomly selected to participate.  All students will be contacted whether or not they are selected for a club.  By submitting this form, your child is committing to attend club sessions.  

SESSION II clubs are scheduled to begin the week of January 4  Additional information, including a Zoom link and specific dates for this club, will be provided by the club sponsor once enrollment is closed.
