This form is to gather contact information for those who wish to tryout for a middle school spring sports team (both boys and girls). Tryouts will begin prior to April Break. The season will end prior to June 1.
Parents/guardians MUST complete forms 1 and 2 and submit to the school nurse prior to March 18th.
The nurse will confirm a valid physical which meets district policy 5141(e). Physicals must be dated AFTER MAY 17, 2023. If your child's physical is very close to this date, you can appeal and we will review to see if we can accept.
Only students who have completed and stamped (approved) forms will be allowed to participate in tryouts.
Approval for participation will be held if a student has a negative lunch balance. All balances must be paid prior to a student being eligible for participation in any Amity middle school or high school activity/sport.
