Photo Release Form

Please sign this sheet (electronically) to give your permission for me to take pictures of your child. There are several reasons I take pictures of your child:
1. To use in the classroom to aid in your child's learning.
2. To show you, as parents, what we are doing in the classroom environment - these pictures may also be posted to our classroom Schoology or webpage.
3. To show how grateful we are when people donate supplies for the classroom. (I occasionally write emails to companies asking them to donate supplies for our classroom. In order to show our appreciation, the students will usually send thank you cards and pictures as well).
