I give permission for my child to attend any/all Friday Together Kids events between April 2021 and January 2022 and I agree to him/her taking part in any/all of the activities involved.

I certify that as far as I am aware my child is medically fit to participate in the activities and there are no known health reasons why he/she should not do so. I give my consent for medical treatment if necessary, to be given in the case of an emergency.

I understand that my child must make every effort to join in with the Together Kids group during the events and fully participate in all the activities.

I understand that it is my responsibility to arrange transport for my child to and from events. I also understand that it is my responsibility to inform the Together Kids team if my child is being picked up by another adult.
