Gunter ISD requests a release of liability on every student that is participating in Summer Camps in Gunter. By checking the box below, students and parents agree to assume any and all risk while taking full responsibility, and waive any claims of personal injury or damage to personal property belonging to their child associated with the Gunter ISD Camp. By giving consent on this form, the parent/guardian will legally waive and release any and all rights and claims for damages they may have against the Gunter Independent School District, and any of its representatives, including all damages which may be sustained and suffered by entry into this camp to include traveling to and from and participating in this camp. The parents have agreed by giving consent below to authorize Gunter ISD to act in the best interest of the camper in event of injury to the student athlete. *
