Dear parent/guardian, Leadore School is requesting your input for teacher evaluations. As required by Idaho code 33-514, IDAPA 08.02.02.120 Local District Evaluation Policy, and Leadore School Policy 5340 certificated instructional employees (teachers) are to be evaluated at least biannually. Multiple measures are utilized in the evaluation process including parental/guardian input. Your input is not only appreciated but is an important piece of the evaluation process.

In an attempt to keep this evaluation as brief as possible, yet maintain integrity and the overall importance of this portion, the district is including the following questions for your consideration and input. We understand that middle school and high school students have more than one teacher, therefore we have attempted to keep the evaluation form as brief as possible.

If you have already filled out an evaluation/input form, we would ask that you please do not fill out another one.
