PDC Course Template Installation Request Form
Please use this form if you would like to have the instructional technologist add the PDC Course Template to your BLANK Canvas course shell. If you would like to add the template yourself, do NOT fill out this form! This is only if you need help importing the PDC Course Template into your blank Canvas course.

Template imports will be processed within 2-3 business days (and sooner whenever possible!).
