Please be aware of the following terms and conditions of this memberhip cancellation:

1. THIS FORM WILL ACT AS YOUR 4 WEEKS WRITTEN NOTICE FOR MEMERSHIP CANCELLATION. Your Membership fees will continue to be debited for 4 weeks from receipt of this completed form, and then will cease indefinitely.

2. Please allow up to 48hrs for this form to be processed. Any debits taken during this time will not be refunded.
If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.

3. If you would like to put your membership on hold instead of cancelling, you can submit a Membership Hold Request for a 4 week period. If you’re sure you’d like to cancel, just complete and submit the form below. This will serve as your 4 week written cancellation notice as required by your membership agreement. Your membership will be cancelled 4 weeks from the submission of the form below. All payments are non-refundable.

4. Upon cancellation of your membership, you will also be removed from any social media groups related to CrossFit 786. Your access to our membership apps, including GymMate and SugarWOD will also be removed as these both form part of a current membership with us.
