Interested in adoption? Please do not apply if pets in the home are not up to date per our website requirements or are not altered. We have recently added Illinois applications to states we will process. Please note, for Illinois applications, as they are out of state, applicants must have no children under 7 years old, must own their home, and must have an excellent vet history. Wisconsin applications do not have these requirements. We do not accept from other states.

**Please Note: Applications with incomplete information will not be processed. Examples include: No veterinarian contact information for pets in the last five years (phone number), no landlord information if renting (name and phone number), failure to include non-family references (non family =no relatives or in-laws), incomplete or excluded birthdates, and incomplete names for adults in the home**

In order to proceed with the processing, all current dogs in the home must be spayed or neutered, up to date on rabies vaccination, up to date on DHLPP, up to date on Heartworm testing and preventative. All cats in the home must be spayed or neutered and up to date on a rabies vaccination or wellness exam. We DO NOT process applications if dogs or cats over 6 months in the home are not spayed or neutered. Applicants with poor vet history will not be processed. Applicants with no history of heart worm testing for dogs will not be processed. BEFORE submitting, please call your vet and release permission for records. Ask your vet if your pets are up to date on all care and listed as spayed or neutered.

Our goal is to place our rescued dogs in permanent, loving homes. By completing this form you will assist us in determining if the dog listed below or another best suits you and your lifestyle. After the application has been processed. it will be forwarded to the homevisit team to await selection. Please note that we are 100% volunteer run and our timeline may not fit what all adopters are seeking. We do anticipate an extended time for scheduling home visits that are not in the Jefferson or Dane County Area. We cannot guarantee that all applications will be selected for a homevisit. All family members and other animals residing in the home must attend a meet and greet with our rescue animal to ensure compatibility. All adopters and co-adopters must be at least 21 years of age.

We will complete a background check. Applicants with concerning items on background checks are not likely to be selected by a homevisit representative. We also invite further information on charges.

Priority is given to Wisconsin adopters and out of state applications may not be processed for dogs with a high volume of applications.

The adoption fee is $250.00 for dogs. If adopting an unspayed or unneutered dog, proof of spay or neuter must be received within 4 weeks post-adoption or when the dog reaches 6 months. If you cannot commit to altering a dog at 6 months, please do not apply. If proof of spay or neuter is not provided, the rescue has the right to revoke ownership and the dog will be returned to the rescue.

No animal will be adopted to prospective owners who mislead or fail to provide accurate information on the adoption application and/or the adoption contract. Our rescue reserves the right to refuse adoption to anyone.

If you are no longer able to keep your rescue dog for whatever reason, the adopter agrees that he or she will be returned to the rescue. The adopter may not attempt to sell or rehome the rescue dog at any point.

Once an application is submitted, the materials are reviewed by the Application Review and Collaboration Team. This team is comprised of members from our sending rescues in Houston, as well as local volunteers and local shelter staff who support best-practice placement of rescue animals.
