Provided that both in person and remote teaching staff are AmPark teachers, would you prefer to start the school year with your child/children enrolled in full time remote learning or in a hybrid model (part time in school and part time remote)? Please note that this is not the official  survey to enroll.  This is being used for school planning purposes.
Clear selection
If you selected full-time remote, but it was not guaranteed that the full-time remote option would be taught solely by AmPark teachers, would you then choose the hybrid model?
Clear selection
If you are choosing the hybrid model, would you prefer your child to attend school in person on (A) a daily schedule with some set days and one or some that alternate or on (B) alternating weeks? For example, your child might come to school Tuesdays & Wednesdays and alternate Mondays with option A, or come to school M-F for week 1 and then be all remote for week 2.
Clear selection
If you have more than one child at AmPark, do you need/prefer your children to attend school on the same days?
Clear selection
Additionally, would you be interested in siblings being grouped together in a mixed grade level group to reduce exposure to as many households for both students and staff members?
Clear selection
If your child is in a specialized program (English Language Learner, self-contained classroom), would you prefer for your child to attend school in-person every day if it were possible?

To prioritize supervised instruction for families most in need as well as to provide one cohesive learning experience for all students, what is your level of support for the following proposal (if it were possible): Majority of teachers and students teach/learn from home.  Students will specialized needs and families with strong childcare needs come to school for in-person supervision and support of the virtual classroom.  For example, Bonnie is a first grade teacher and Kelly is a first grade student with parents that need to work full-time during the traditional school day.  Niomi is a first grade student that can stay home for remote instruction.  Bonnie teaches both Kelly and Niomi remotely.  Niomi logs on from home.  Kelly logs on from the school building while Casey supervises and supports her first grade learning.

Are there ways that you are able to help with the reopening process? Examples include supporting translation, helping other parents with tech support, donating/securing donations for additional non-provided PPE supplies such as tents/gowns/etc., making masks/earsavers/lanyards, contacting local organizations to secure outdoor space/provide remote instructional programs, taking temperatures at arrival, helping to maintain social distancing at arrival/dismissal, working on childcare options for families on remote days, walking children other than your own to/from school to reduce busing, etc.
What suggestions do you have for safely reopening schools?  Please be mindful of CDC guidelines and recommendations in suggestions.
What concerns do you have?
Anything else that you would like to share?
Is there anything that you need?  Tech, wifi, food services, family support services, etc.
