Waiver Release:  My child is in good health and has my full permission to participate in all lacrosse activities set forth by camp staff.  I fully understand that lacrosse is a contact sport and that physical injury may occur during the course of practice and games.  A Plus Lacrosse LLC  and the Sports Domain Academy assume no responsibilities in the event of an injury.  Accordingly, I accept all risks and will waive all claims arising out of the activities of this program on behalf of my child.  I also understand that A Plus Lacrosse LLC  and the Sports Domain Academy are not responsible for the loss of any personal belongings. *
