Daily Home Screening - English
Starting every single morning, each of our community members plays a vital part in keeping our students safe. Before sending students to school each day, caregivers will need to evaluate their student(s) with this Daily Home Screening. Student(s) must stay home IF:
- The student(s) shows any signs of illness.
- Anyone in the home has a confirmed or presumptive case of COVID.
- The student(s) has been exposed to someone with a confirmed or presumptive case of COVID.
- Anyone in the home is waiting for the results of a COVID test.

Please work with MITCH to prevent the spread of COVID-19 in our community. If students have any symptoms of illness, they will be promptly isolated from other students with support from an adult, and sent home for the health and safety of all.

If you answer YES to any of the following four questions, DO NOT bring your child to school.
1. Within the past 14 days, has your student been in close physical contact (6 feet or closer for a cumulative total of 15 minutes) with: Anyone who is known to have laboratory-confirmed COVID-19? OR Anyone who has any symptoms consistent with COVID-19? *
2. Is anyone in your household isolating or quarantining because they may have been exposed to a person with COVID-19? *
3. Is anyone in your household currently waiting on the results of a COVID-19 test? *
4. Does your child have ANY of the following symptoms? *

If your child has ANY of the above symptoms, keep them HOME from school. If you answered YES to any of the above questions, keep your student HOME from school.
Contact the school office to report your student's absence. The school may ask some additional questions to help determine when it is safe for your child to return to school.

If your child does NOT have any of the symptoms above, and has answered NO to the questions, send them to school as usual.
