-The student needs to answer "yes" to question(s) on the Daily Health Form
-The student is scheduled to attend a rehearsal date on campus
*Note that certain "yes" answers, such as routine testing or traveling with immediate family by car, do not necessarily disqualify you from attending rehearsals. Please email the director before completing this form about your specific situation if you are unsure.

Please follow the current CDC guidelines regarding dates of exposure, testing, etc. as they apply to your student missing or attending rehearsals. Err on the side of caution, but please be honest as well. We do not allow students to receive attendance credit for non-COVID related reasons such as running late, conflicts with other activities, or for failing to complete this form on time. An absence request form is still required for non-COVID related absences from rehearsal. Once you complete this form, you will receive an email with further instructions about receiving credit for the specific rehearsal.

This form must be completed by the parent/guardian as soon as you become aware that your student will be unable to attend an on-campus rehearsal and no later than 3:00 pm on the date of the rehearsal.

Completion of this form is required in advance in order to receive virtual attendance credit. All answers on this form will be kept as confidential as possible but will be reported to Foothill Christian School or to the state health department as necessary.
