Sir Wilfrid Laurier PAC is putting together a Friendship Directory for Laurier parents and guardians/other caregivers. This is a great resource for contacting other members of our school community.

The directory is a printed list with student names and parent/guardian contact information. It will be ordered by division, with each division ordered by student last name. You can list one or more parents/guardians under each of your students that attend Laurier. For each parent/guardian you can list a phone number for parents to call, a phone number for Laurier students to call, and an email address.

The Friendship Directory is delivered as a multipage printout. One will be sent home with each student at Laurier.

To have your information published, you must fill out this form. Each parent/guardian will need to confirm understanding that any contact information submitted in this form will be included in the Friendship Directory.

You can also download a fillable PDF of this form on the PAC website at https://laurierpac.ca (see link on the home page) and return it to the PAC mailbox in the school office (if you send with your student, it will find it's way into the box), or email the electronically signed form (PDF signing is accepted) directly to laurier.pac@gmail.com.

The deadline for form submission (either this form or the PDF) is October 29th, 2021.
