20-21 Operation School Bell Request Form for Owyhee Elementary        
Operation School Bell is a project of the Assistance League of Boise. They assist families by providing new clothing and shoes to elementary school students without cost to families.

They will make every effort to select clothing that fits your child based on the measurements and sizes you provide below and what they have available.

ACCURATE MEASUREMENTS AND SIZES ARE CRITICAL, as returns and exchanges are NOT ALLOWED due to Covid-19.

Please fill out the form below for EACH CHILD at Owyhee Elementary you wish to participate. Contact your school counselor, Mr. Diemart, charles.diemart@boiseschools.org if you have any questions. All information is confidential.

Sewing measuring tapes are pretty inexpensive at Walmart/Fred Meyer, etc.  If you cannot afford one, I may have one that I can send you (email me to let me know), but you'll have to wait for me to get my delivery and then send them to you.  If you can borrow one from a friend or family member who sews, that may be the fastest way to get the information needed for this form....

FOOT TRACING EXAMPLE for Hard-to-fit feet:
By pressing SUBMIT, I know I'm REQUESTING that my child receive clothing from the Assistance League of Boise.  Pressing SUBMIT also PROVIDES PERMISSION as PARENT/GUARDIAN for my child to receive clothing from the Assistance League of Boise.  Because spaces in the program are limited, I know Owyhee Elementary will do their utmost to honor parent requests and will contact me regarding the status of my request once they have that information.
