By signing the above, the person attending the meeting confirms that he/she and (where applicable) his/her child(ren) are attending the meeting freely and voluntarily, without any force or undue influence. He/she further confirms that all the information given herein is, to the best of his/her knowledge, true and correct as at the time of entering the venue / attending the meeting. He/she agrees to comply with the social distancing, health and other protocols as required and communicated by the Church, and further notify the Church if he/she/their child(ren) should test positive for COVID-19 within ten (10) days of attending a meeting at Church. He/she indemnifies One Hope Church, its religious leaders, employees and/or agents, against any legal claim of whatsoever nature in the event that he/she/any child or person living with him/her, should test positive for COVID-19 at any time. The information contained in this form will be treated as private and confidential and used solely for the purposes of screening and contact tracing.
