In case of inclement weather, we dismiss at exactly the same time as the public schools. The announcement of such a dismissal is broadcast on the radio/TV and a phone call from the school messenger system, as soon as the decision is made. Any other unexpected early dismissals will also be noted on the radio/TV/school messenger. We need to know your wishes as to your children's transportation home. In the event of an unexpected early dismissal, my wishes concerning my children's dismissal are: *

I understand the dress code and uniform policy for K-5 and will not send my child (ren) to school with logos on their clothing unless it is a spirit day or choice clothing day. No student is allowed to wear shorts to a school Mass. Preschool students do not follow the uniform policy. *

Permission for Field Trips: *

I give permission for my child(ren) to participate in activity/field trips outside the school grounds as scheduled unless I advise the teacher differently, in writing or a particular proposed trip. I understand that any excursions will be supervised by a teacher who shall exercise due care and caution in providing for the safety of his/her students while on such excursions.

I understand to volunteer or be a driver for field trips I must complete the safe environment training and fill out the driver form 72 hours prior to the volunteer commitment or field trip. If these items are not taken care of prior then you will not be able to drive or volunteer. Once complete these are good for the whole school year and must be renewed each school year.

Internet Permissions: *

I give permission for my child(ren) to use the internet at school. I understand that a member of the faculty staff, volunteer or parent/guardian will be present when the students are using the internet.

To ensure the South Dakota Department of Health is aligning with the Health Insurance Portability and Accountability Act (HIPPA) Omnibus Rule, a School Health Official must obtain parent, guardian or legal representative agreement before accessing a student's immunization record in the South Dakota Immunization Information System. No student record shall be accessed by a School Representative in the SDIIS without parent, guardian or legal representative agreement. I give a St Agnes representative permission to access the above children's immunization record in the South Dakota Immunization Information system *

I understand that Red & Gold before/afterschool care, preschool tuition, K-5 school tuition and lunch money are different payments. A monthly statement from Red & Gold will be sent at month end, checks should be made out to Red & Gold. On registration/meet the teacher night you will set up the P-5th school tuition payments and make payments as agreed upon, checks should be made out to St Agnes School. If school payments are not completed prior to May 20th, the student will not receive their last report card & 5% late fee will be placed on the account for the remaining amount. Once payment in full has been made and any late fees paid the child will receive their report card in the mail. *

St Agnes will be charging $3.05 per meal this school year. If you need a lunch application for FREE or REDUCED prices to help. Please print the application on our website or contact Renae for a copy. Checks can be made out to St Agnes School and the word LUNCH in the memo.

St Agnes School is able to keep tuition at a minimum due to the service that parents provide with our fundraisers. Parents are strongly encouraged to perform a minimum of ten (10) hours of service per family in time or talent by helping the school in different capacities. Our greatest need is service at fundraisers; specifically planning, setting up, serving and clean up. *
