ATTENTION:  This form is for students enrolling in Tucumcari Middle School or High School ONLY.  If your child needs to enroll in Tucumcari Elementary School, please CLOSE THIS FORM and contact 575-461-8460 (PK-2) or 575-461-8461 (3-5).

Tucumcari Schools have switched from TYLER to POWERSCHOOL for grades, attendance, etc.  At this time, the online registration through PowerSchool is unavailable, so we need the information requested in this form.  Some questions  require a signed document which will be available during in-person registration.

Families will complete this form to VERIFY and UPDATE their child's enrollment information as needed.

Questions marked with a RED ASTERISK must have a response.
