We welcome all new children to our Religious Education program. Please complete this form and we will be in touch.

NOTE -- Our First Communion and Confirmation programs are two-year programs. First Communion spans First and Second grades and Confirmation spans Seventh and Eighth grades. If your child is starting a bit later that's ok -- we are happy to work it out with each family. At the end of this form just add your specific circumstances in the COMMENTS section. You can be assured all information provided will be kept confidential.

If you have multiple children we ask you to complete a separate form for each.

PS -- If your child has attended any religious education classes at St. Eulalia they are automatically enrolled. You do not need to complete this form. However, if your mailing or email address has changed please contact Mrs. Clancy with that updated information (religioused@saint-eulalia.com)
