NEW OPTIONAL FEATURE: Choose your own team. Want to create your own team of family members, friends, social group, church group, work crew, etc.? Now you can! IMPORTANT: Each team must consist of at least 4 members by the time the contest begins in order to remain a separate team. If the team contains 3 or less members by the start date (May 31, 2024), it will be absorbed into whichever library team has the least number of teammates. If this happens, we will inform you of the change via email.

If you are signing up minor dependents, you MUST be their parent or legal guardian, as completing this form electronically constitutes an agreement to the terms of the contest between yourself and Fort Scott Public Library. Independent adults should each fill out their own form.

Your daily step count can be captured via smart phone, smart watch, pedometer, or a similar digital step counter. If you do not know, you may give us your best guess.

Highly Active - More than 12,500 steps per day on average

Active - More than 10,000 steps per day on average

Somewhat Active - Between 7,500 & 9,999 steps per day on average

Low Active - 5,000 to 7,499 steps per day on average

If you have a Discord account, please list your Discord tag (username) here. Use of Discord is NOT required in order to participate. However, we hope you will take advantage of this opportunity to communicate with other participants.

We will create a special Discord channel for each library team, and participants 13 and older will be invited to join the library's server to chat with other participants. The library's Discord server is a private, moderated online space for members of library programs only. If you would like to create an account and join the library's server now, use this code: https://discord.gg/msk9abRu You may use Discord in any internet browser, install the desktop program, or download the free app. This would be a great way to keep in touch with all of your teammates, so conspire together to sign up!

You may log your steps on ReaderZone.com or on their app. We also have a Google Sheets option. Which would you like to use? NEW: Reader Zone now offers the ability to enter a 5 digit-number, so you will be able to enter all your steps up to 99,999! The contest will be set up in a more logical manner this time in Reader Zone, thanks to new features just released in the app! We encourage you to try it out. *

PLEASE only choose the Google Sheets option if you plan to check your email for the link and to use Google Sheets on a regular basis for logging your steps. Miss Val must manually copy & paste each entry into Reader Zone, which takes a lot of time & effort.

A t-shirt with the program logo on the front will be provided to each participant, while supplies last. Pick yours up on or after the first day of the contest (May 31, 2024).

I understand that...

Exercise is not without its risks and this or any other exercise program may result in injury. As with any exercise program, if at any point during my workout I begin to feel faint, dizzy or have physical discomfort, I should stop immediately and consult a medical professional.

I agree to report my actual daily steps, NOT estimates, as recorded by an electronic device such as an app, smart watch, or pedometer, in the Reader Zone app or website.

I should always consult my physician or healthcare provider before beginning any nutrition or exercise program. If I choose to participate in this program without prior consent of my physician, I am agreeing to accept full responsibility for my decisions and agreeing to hold harmless Fort Scott Public Library, Bronson Public Library, and Healthy Bourbon County Action Team, their agents, employees, contractors and any affiliated companies, from any liability with respect to injury or illness to myself, my legal dependents, or my property arising out of or connected with my participation in this program.

If the program administrators suspect cheating, either by manipulation of the Reader Zone app or of reporting an inaccurate number of daily steps, the program administrators reserve the right to challenge and/or remove my reported step count. If I believe the administrators have removed my step count without cause, I can appeal by stating the reasons why I believe their actions were unmerited in an email to vcyouthlib@gmail.com.

The program administrators are happy to help me and to make necessary accommodations for my participation in this contest. It is my responsibility to ask for assistance when needed.
