This information will only be used to meet those requirements and may be produced to legal authorities on request. By preregistration for the training session, you consent to the information you've provided being accessible to the Constabulary of Innilgard, the Seneschal and, if required, SA Health for the purposes of contact tracing and membership management.
You will need to fill this form out for each person who will be attending. If you are filling it out for a minor put the name and contact details for the child (eg their parent's contact number and email address - if known) into the next page and select the correct membership type. Put the attending parent's name in the subsequent page.
