Please carefully review Neshannock Township School District's Reopening Plan posted at NTSD.org prior to completing this form. A hard copy of the plan has also be mailed to the parents and caregivers of all students currently enrolled at NTSD.

As Neshannock Township School District prepares for the 2020-2021 academic year, we respectfully request that parents and caregivers complete this electronic form ONE TIME FOR EACH student -- Grades K-12 -- in their household. If you have three children attending school in 2020, you will complete this form three separate times.

Please complete and submit this form -- ONE TIME FOR EACH STUDENT -- by August 12, 2020.
