You may pay using a credit card or a check; we do not accept purchase orders. Payment must be made by February 15, 2022 to take advantage of Early-Bird Registration.

Due to restrictions of Google Forms, you can't save this form. So, before clicking "Submit", you must print a copy of this form for your records. There is NO NEED to mail the registration form with your check because information is saved in our Google Spreadsheet as you complete the registration form.
