Attendee Information 
Please provide as much information as possible about the people in your group. This will help us to be prepared with age appropriate activities, food, t-shirts etc. so that we can make this a special day for everyone involved. Thank you!
Children attending under the age of 13 (please list name, age, shirt size)
Teens attending ages 13-17 (please list name, age, shirt size)
Adults attending ages 18+ (please list name, contact email or phone, shirt size)
We would like to provide as many people as possible with a custom walk t-shirt. There is a suggested donation of $25/shirt or $100 for a group of 5 or more. If you set up a fundraising page and raise at least $100, t-shirts will be provided for your group. If you would like to set up a walk team or make a donation click here. Donations will also be accepted at the event, however, t-shirts not reserved prior to August 5th will be available on a first come first serve basis.
