Please complete the form below to request a change to your students' delivery model for Quarter 2. The deadline to complete this form is Friday September 18th. Please note, our intention is to minimize schedule changes but as our students change instructional models, we must change teacher assignments to meet the needs of our students. As you have heard many times now, as we continue to pivot during these unprecedented times, flexibility and understanding is needed and appreciated.
We will contact you regarding the change as we get closer. This change will not go into effect until October 13th.
