By submitting and signing this form, I, a parent or guardian of the participant, acknowledge that I am aware that there are risks associated with gymnastics. I warrant that the participant named on this information form, is physically fit to participate in gymnastics. I declare that I have accurately disclosed all information regarding physical, mental or medical conditions affecting the named participant and acknowledge that this information may be used for the Gymalaya Inc. use in the delivery of a gymnastic program. I acknowledge that there is potential risk for injury and/or contracting COVID-19 or any other contagious disease involved in training and competing in any sport/activity. I understand that Gymalaya Inc. has tried to create a safe and controlled environment for participation and that it has established rules for participation on and about the gymnastics area that must be followed by the participant. I understand that failure to comply with any of the policies and rules of Gymalaya Inc. may result in the suspension or termination of membership. I waive the rights of the participant to damages or other costs in the event injury is caused due to participation in gymnastics. I understand that it is my responsibility to ensure that the information on this form is kept current and I will notify Gymalaya Inc. of any changes immediately.

The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization and COVID-19 is extremely contagious. Gymalaya Inc. has put in place preventative measures to reduce the spread of COVID-19; however, Gymalaya Inc. cannot guarantee that the Participant will not become exposed and/or infected with COVID-19, and such exposure/infection may result in personal injury, illness, permanent disability, or loss of life.

In addition to giving this full consent for my child's participation, I do hereby agree to waive any claims and/or rights that I might otherwise sue Gymalaya Inc., its officers, directors, shareholders, employees, agents and all others associated with the corporation from all liability for any and all damages and/or injuries that might occur as a result of these activities.

General Information

Everyone is required to remove their shoes upon entering the facility. 
All children under the age of 4 must have a parent with them inside the gym at all times.
Adults are required to wear socks in the gym. Kids are encouraged to wear non-slippery socks or go barefoot in the gym.
Parents are not allowed on the equipment at any time.
Alcohol is NOT permitted in the facility.
No nuts/peanuts are allowed inside the facility.
Please clear the facility at the end of your party / class to allow us to clean and disinfect before the next party arrives.
Please do not leave valuables unattended. We are not responsible for lost or stolen items.
