Parent/Guardian,
Please complete the following application if you wish for your child to be considered for the SCSD "At-Home/Virtual Learning Plan." This application consists of two parts to be completed by the parent or guardian. If you have any questions, please contact Mr. Victor Gilstrap at vgilstrap@scott.k12.ms.us or 601-775-3248.

NOTE: Please answer all questions in section 1 of this two-part form. If you respond "yes" to all three questions in section 1, please proceed to section 2 of the questionnaire.

Can you provide documentation that your child, or anyone in the household, has an underlying health condition which identifies them as "at-risk" according to CDC Guidelines? *
