This form must be completed by the registering participant's legal guardian. If registering multiple participants, please complete a separate form for each youth.
SHIFT registration is always open and we will follow up with you. However, we only accept new participants for the first three weeks of each rotation. After that, your child will be referred to the upcoming rotation.
The Muncie community is excited to host SHIFT, a grief group for youth grades 3 through 12, at Commonway Church.
Our next cycle will run Sept. 16th - Nov. 18th, 2024 6-8pm. (Please be aware there will be no SHIFT Oct. 7th due to fall break.)
