MEDICAL INFORMATION

Please include any Behavior/physical problems/handicaps/diets/headaches/anger/etc.

(This is very important for us to be aware of so we can make sure the time your child spends at camp is fun and safe ) Please also include any triggers/possible issues that your child might have. Additionally, please let us know things that help your child if a situation arises(i.e. ways to cope or calm them down.)

Allergies (food or medication)-Breakfast, Lunch, & Snacks provided daily-You may choose to pack if your child has a food related allergy

My child will bring the following (Please know that ALL prescription medications must come in its original container, with doctor's label. They will be turned in at the registration and administered by the Healthcare staff for the week. Children who are sick or too weak will be sent home.)

IN CASE OF EMERGENCY

I, being the parent or legal guardian of this child, give Hilltop Christian Fellowship permission to act in my behalf in giving informed consent for medical treatment for my child when I am unavailable to give such consent on June 24th through June 28th 2024. I also agree to pay for the performance of such treatment as deemed necessary in the opinion of the attending physician.

INFORMATION FOR YOU TO KNOW:

**Camp start time is 9AM/Doors open at 845AM Daily & Pick up is at 4PM

**We will offer a hot breakfast, lunch, & daily snack (Menus are available upon request) You are welcome to send your child with packed lunch also.

**It would be a good idea to bring an insulated water bottle that can be refilled throughout the day & apply sunscreen daily.

**Camper t-shirts will go home Monday, the first day of camp, your child may choose to wear their shirt any day of the week, but we ask ALL campers to please wear their shirts on Friday

**We will going off site from HCF two times throughout the week, On Wednesday to Potomac Parks & Retreat Center in Falling Waters & on Thursday to Camp Harding in Big Pool for baptisms.

**Please let us know about early dismissals in the morning at drop off.

**Deadline to register is May 21st, please ensure payment is received by this date or registration can't be processed
