Incident Reports requires you to submit your name and a detailed account of the incident including all of the specific details. However, you may choose to remain anonymous and all personal identifying information will be confidential.

This may be formally investigated by the Judicial Review Committee and appropriate actions will be taken to handle the situation. The information you provide may be reviewed by the Judicial Review Committee.
