Every staff member has to answer the questions in the APS Employee Questionnaire and take their internal temperature before reporting to work.  

In the past 24 hours, you have experienced the following:
Fever: Yes or No
Fatigue: Yes or No
Frequent, Dry Cough: Yes or No
Aches and Pains: Yes or No
Chills: Yes or No
Sore Throat: Yes or No
Nausea or Vomiting: Yes or No
Diarrhea: Yes or No
Headaches: Yes or No
Congestion or Runny Nose: Yes or No
Shortness of Breath or Difficulty Breathing: Yes or No
Loss of smell or taste in the past 24 hours: Yes or No
Have you recently been in close contact with anyone who has exhibited any symptoms? Yes or No
Have you recently been in contact with anyone who has tested positive for COVID-19? Yes or No
Do you currently have a temperature over 100.4 degrees Fahrenheit?
Have you traveled out of state in the last two weeks?
Have you been in contact with anyone who has tested positive for COVID-19?

Confirm you have done this each school day by 7:30 am by finding your name below and answering the question.

If your internal temperature is greater than 100.4 degrees Fahrenheit or you have answered "yes" to any of those questions, please call or text Dan immediately and DO NOT come to school. He will give you information on the next steps.

If you do not complete this questionnaire and take your temperature prior to arriving at school, you will need to report to the nurse's office for a temperature check and complete the questionnaire.
