I hereby release Stars R Us Dance Studio and all their employees from all claims on account of injuries, which may be sustained while attending class, performances, special events, and/or special practices. I also give Stars R Us Dance Studio permission to photograph the student while they are participating in studio related activities I, the guardian, have read, understand, and agree to comply with the studio's policies and procedures. *
