DUCC Stephen Ministry Survey, Closes June 27
Pastor Sal and our Stephen Ministers want to be sure they are meeting the needs of hurting church members. This survey is designed to explore our understanding, awareness, and ownership of the ministry, and to find new opportunities to serve. Your participation in this survey will help us do just that!

The survey can be completed in just a few minutes. Please complete it by June 27.

This survey explores four important goals for our Stephen Ministry:
1. Knowledge: People understand what Stephen Ministry is all about.
2. Passion: People are proud of and feel ownership of our Stephen Ministry.
3. Calling: People understand that we are gifted in and called to involvement in the ministry.
4. Receptivity: People are open to receiving care when they need it.

Instructions:
The survey contains statements related to each of the four areas above. Below each statement is a scale numbered 1 to 10, where  1 means "Not at All True," and 10 means "Very True."

Respond to each statement by choosing a number from 1 to 10. A number between 1 and 10 shows how far you lean in one direction or the other.

Below each statement is room for comment. Feel free to share your thoughts or comments about any of the statements or your ratings.

Thank you very much for your time and thoughtfulness!

With love,

Pastor Sal and Your Stephen Ministers
