King Athelstan Primary School Child Medicine Form 2023/24
All medicines must be in their original container with prescribing information that includes the name of the medicine and the dose, and your child's name.  Any medicine that does not include this will not be accepted into school.

King Athelstan will not accept over the counter medicine, except Antihistamines, Paracetamol and Ibuprofen, recommended by a doctor. If your child requires regular doses of over the counter medicine e.g. Paracetamol for pain relief, a letter must be provided by a healthcare professional in order for this to be administered in school. The school will only administer antibiotics if they are required at least 4 times per day.

Medicines should always be handed into the Office.  For medicines being used for short-term purposes, please collect your child's medicine after school.  For medicines being used for long-term purposes, please collect your child's medicine at the end of each term.

Please complete a new form each time your child has a change of medicine.
