The purpose of this form is for you to provide a brief outline of the Community Service and Campus Outreach events that you are planning for your chapter. We hope that having this events schedule will encourage your chapter to reach your desired goal and do so with more ease throughout each semester!

Please provide your plans for the upcoming semester at the very least. You do not have to strictly abide by this schedule, but you should use it as a general plan for the semester/year. Please provide the name, date, description of each event, and roughly how many service hours it will require to complete. If you are having trouble coming up with ideas for your event, please contact your appointed Regional Student Leader for Campus Outreach or Community Service or check the chapter resources page on the OCCM website for a list of ideas to do in your chapter!
