Please complete this form for each fundraising activity you are planning. You will receive a follow up from Holly Dickinson, Director of Grants and District/Community Programs, to make sure that your fundraiser is on the Fundraising Calendar and that there is online access to information about your fundraising event on the District website. Please remember to send Holly the approved & signed Principal/Director Fundraiser Approval Form either via email or as a Google Doc link under Additional Information at the bottom of this form.
