Feel free to fill this out completely anonymously, none of the questions are required. We are not collecting email addresses unless you want to provide it.

Casandra Roberts, Steven Meyer and Shannon Tetley are members of the FPI community and on the Safety Leadership Team. They will receive all reports filed. FPI owners will also receive notice of the complaint. If anyone on the Safety Leader Ship team are part of a complaint they will be asked to recuse themselves from the process.

If you provide your name and contact info an initial acknowledgment of the issue will occur within 24 hours, and we will make available time within 72 hours for an initial discussion, though we may mutually decide to wait longer to have that meeting.

If you do not provide your name and contact info we will act in good faith.

In the rare instance that the filed report is beyond our scope FPI may involve law enforcement or other agencies.
