2019 Awards Recital Performer Information

This form is for collecting PERFORMANCE information only (for the event program). Please repeat this form for EACH piece your child will perform.

To REGISTER your child as a perfomer, please use the online calendar registration (visit the Parent Portal on our website www.funmusic4u.com). Click on the Calendar icon and scroll forward to find June 2nd. Click on the Recital event, then click "Register" and select your child's name from the dropdown list.

All are welcome to attend the Recital! Please invite friends and family to support your child's performance! Only performers need to be registered online, however.

Name of Performer as it should appear on the program and awards/certificates. *

Write your child's name if the piece is their composition. If the piece does not have a specific composer, please note if it is a Traditional, folk song, etc., or alternately the name of the method book used, if applicable.

Will a soundtrack be required for the performance? *

Yes, photos/videos of my child at the 2019 Award Recital may be used for Fun Music School's promotional efforts.

I understand that the $40 registration fee for each performer is non-refundable once awards have been ordered and that any awards will be presented by the teacher on a following lesson if the student misses the recital for emergency reasons.
