Call for Nomination 2021
Please complete and submit this to the Academy Office by August 25, 2020

The Academy of Hand and Upper Extremity Physical Therapy invites its members to answer the call for nominations for elected positions within the academy’s leadership. We are looking for strong, energetic, proactive leaders to join our team. Please consider the following positions:
• Vice President (1 to be elected; 3 year term)
• Secretary (1 to be elected; 3 year term)
• Director (1 to be elected; 3 year term)
• Nominating Committee Member (1 to be elected; 2 years as member and 1 year as chair)

Nominate an Academy member or yourself that is in good standing (at least 2 years for VP, at least 1 year for other offices).  *Be sure to indicate which position you are nominating someone for.  Elections will occur by electronic ballot, and the newly elected will be announced within 3 weeks of the close of the election.  Those elected will assume office at the Annual Meeting (CSM 2021).

This form can be submitted in part or whole yet all information is required by August 25, 2020.
