2023-2024 Weekly Formation Registration
Please use this form to register your family for Weekly Formation. For simplicity we will be grouping these programs on this registration form.

Age 4 - 5K: Junior Disciples - Meet Sundays during Mass
Grade 1-5: Youth Formation - Meet Wednesdays 3:30-4:30
Grade 6-12: Teen Formation - Meet Wednesdays 6:30-7:30
Sacrament Preparation - Class Schedule TBD

Each age group will have a Family Letter sent home each week.  The letter will explain their topic of the week and ask families to review and continue the teachings at home. Questions on the Family Letter can be addressed to the Coordinator of Faith Formation.

There is limited flexibility with Family Formation in that we are not planning to transfer families between the two programs.  Curriculum will be ordered based on where you register your children for Formation.  Please make the best decision for your family, but please reach out if anything changes.

We are currently selecting the curriculum with our new Pastorate. Once the curriculum is picked, the Pastorate will agree to a fee schedule to best cover material and supply costs for the year.

Weekly Formation Families will have 4 mandatory Family Events throughout the school year. Adults and Children will all receive age-appropriate formation on a similar topic during each of the events. Any parish adult or family is invited to these events as well as the meal/social that will take place.

The idea behind these events are similar to the "Generations of Faith" program that we used many years ago. These events will bring our families together to learn and socialize. This will also give us a chance to further engage with our Parish Families.

This move is phase 1 of our transition to offering only Family Formation as of the 2024-2025 school year based on the Diocese of Madison and Bishop Hying's request and guidance.  We are offering this transition year. It is during this year that we will determine how our Weekly Formation Programs best fit into Family Formation or one of the many hybrid models already approved by the Diocese. 
Knowing that your family must attend 4 events, when would you prefer it to be?

Our mass times beyond June 30 are TBD. Formation meetings based around a Mass will be scheduled before or after Mass depending on the Mass time.

If any child (or parent) has dietary restrictions, allergies, or medical diagnosis, please list their name and what alert you would like to maintain in the Parish Office. For privacy and safety, only classroom relevant concerns are noted to Group Catechists. (note: genetic or neurological disorders that may impact their classroom life should be noted)
Volunteers Needed!
We are looking for volunteers to help make and serve for the social/meal time. Please consider offering for your family to act as kitchen or serving help at least once this year. We are not listing the actual signup, we are just asking you to pray on the potential.
