Kindly complete this form to withdraw from the aquaDucks swimming programme.

  • Only withdrawal forms will be accepted as an official notice.
  • Kindly submit the withdrawal form by the 10th of the month to avoid automated charges on your card/account.
  • Failure to submit the withdrawal form within the above time frame will result in a forfeiture of deposit.
  • Email, SMS, or verbal notice of withdrawal will not be accepted.
