Declaration of Good Health and Travel History - Students travelling to the UK
Please complete and submit this form in advance of your course at IH Bristol.

International student travel is one of the sectors affected by the Coronavirus outbreak. Please carefully answer the questions below, to help us minimize the risk of spreading the CoVid-19 virus.
Coronavirus disease (CoVid-19) is an infectious disease caused by a newly discovered coronavirus. Most people infected with the CoVid-19 virus will experience mild to moderate respiratory illness and recover without requiring special treatment. Older people, and those with underlying medical problems like cardiovascular disease, diabetes, chronic respiratory disease, and cancer are more likely to develop serious illness (WHO July 2020).

The main symptoms of coronavirus (UK National Health Service July 2020) are:
• a high temperature
• a new, continuous cough - this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual)
• a loss or change to your sense of smell or taste – this means you've noticed you cannot smell or taste anything, or things smell or taste different to normal

Most people with coronavirus have at least one of the symptoms above.
If travelling to the UK and/or booking an IH Bristol course, we recommend you have travel insurance before you travel to the UK.
IH Bristol offers student insurance for round trips (in which you travel to, and then depart from the UK). You can find full details here: https://www.ihbristol.com/english-courses/international-student-travel-insurance
