Complete this form to select the type of booth your organization requests for the 2022 Horn Lake Back-to-School Bash on Saturday, August 6, 2022.

Booth space is limited and will be filled on a first come, first served basis.

All vendor booths are required to provide a fun engaging activity during the event.

We also encourage organizations to decorate their exhibit area; however, all materials and displays must follow DCS school policy, be age appropriate and family friendly.

To ensure we allocate sufficient resources, please submit your form and payment no later than Friday, July 29, 2022.

Option 2: Healthcare - Direct Service Provider (providing free haircuts, immunizations, etc. to students during the event) (Cost - Free)
