Enclosed please find a 2022 Corn Hole Fundraising Event Vendor/Food Truck application for participation. Our Fundraiser is a one-day event at Cantine Field in Saugerties, NY.

The date of the event is Sunday, May 22, 2022. It will be held from NOON until 5:00PM.  You will be able to setup earlier day of.

We will only be charging $60 per food truck and vendor tables. If you need electrical, this will be an additional $40. We also ask that you provide a raffle as well – value of at least $20.

Donations for this event will benefit the Athletes of Special Olympics New York Hudson Valley Region.

Safety is important; your tent/canopy must be securely anchored to prevent it from becoming a dangerous, airborne projectile, due to the potential high winds.

Use of the trademark (Special Olympics New York) must be approved prior to using.

FOOD VENDORS ONLY: Please also note, that all teams (2 players per team) will be given two tickets for drinks and two tickets for food. Please keep track of these tickets and we will reimburse you after the event. THANK YOU!

Please sign & return to the address indicated on cover letter (Pg. 1). A copy will be returned to you by Teresa Gilli, Director of Development (via email) & for your files.

THIS AGREEMENT made this _______ day of _____________ 2022, between Special Olympics New York herein referred to as “Corn Hole Management Team” and _____________________________ vendor/food vendor space for the 1st Annual Corn Hole Fundraiser for Special Olympics New York – Hudson Valley Region to be held Sunday, May 22, 2022, at Cantine Field in Saugerties, NY – event is from NOON until 5PM.

Food Vendors – we will be giving each player a ticket. Please provide them with a drink and meal. Please keep track of tickets and we will reimburse you at end of day. Thank you!

Each vendor must bring their own tent, table and Chairs.

Total rental fee for the one day is $60 and is to be paid to the Special Olympics New York (additional fees for electrical if needed – extra $40)

PLEASE TAKE NOTE TO HAVE THE BELOW ON HAND JUST IN CASE YOU ARE ASKED TO PRESENT THESE – ALSO FOOD TRUCKS, BE PREPARED INCASE AN INSPECTOR SHOWS UP. IT IS A PUBLIC PARK AND JUST GIVING YOU ALL A HEADS UP!

NEW YORK STATE SALES TAX: Vendor/Food Truck must possess a valid N.Y.S. Sales Tax I.D. Number. Please, display during the Corn Hole Fundraiser from NOON – 5PM just in case it is needed

AFFIRMATION AND LIABILITY RELEASE

I, hereby affirm that I have been well advised and thoroughly informed of the inherent hazards and policies of the event. I know that by participating in the 2022 Corn Hole Fundraiser, I am exposing myself to certain known and unknown liabilities in my direct and indirect actions with the public and invitees of this event. I also understand that if I choose to hire help for the event, that I am directly responsible for their actions or injury. I hereby personally assume all risks associated with my voluntary participation in this event for any harm, injury or damage that may befall me or any employee or temporary help, as a result of my participation, whether foreseen or unforeseen. I understand and agree that Special Olympics New York, Cantine Complex in Saugerties, NY its members and/or directors and officers may not be held liable in any way for any occurrence in connection with my participation in the that may result in injury, death, or other damages to me or my family, heirs, or assigns, and in consideration of being allowed to participate in this event, I hereby personally assume all risks in connection with said event for any harm, injury, or damage that may befall me, including all risks connected therewith, whether foreseen or unforeseen; and further to save and hold harmless said event and persons from any claim by me, or my family, estate, heirs, or assigns arising out of my participation in this event. I further state that I am of lawful age and legally competent to sign this affirmation and release, or that I have acquired the written consent of my parents or guardians; that I understand the terms herein are contractual and not a mere recital; and that I have signed this document of my own free will.

It is my intention by this instrument affirmed by my signature below to exempt and release Special Olympics New York, Cantine Complex, Saugerties, NY its members, directors and officers from all liability whatsoever for personal injury, employer’s liability and workers’ compensation, property damage or
wrongful death arising out of or in the course of my participation in this event.

I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS AFFIRMATION AND RELEASE BY READING IT BEFORE I SIGN IT.
