Book Cover Form
Select your package and fill out the details of your project to schedule your spot. Filling out this form indicates you are ready to sign a contract and pay the invoice. If you have questions before beginning, please contact me first. Otherwise, complete one form per cover and answer N/A for questions that do not apply. If selecting a paperback or dust jacket, clients are responsible for providing the template(s). They can be downloaded for free through the Amazon Template Generator or through IngramSpark Cover Template Generator.

It's ok if you don't have all the details/requirements as that can be provided by the start date. At the minimum, please submit your name, book title and PayPal email. Thank you!
