Entry form for the 2022 KAA Sweepstakes.  

Please read the form carefully!

Please fill out one form for each owner.  

You are required to enter information where there are red stars.

Only animals with valid registration numbers may be entered online.  Animals with pending registrations must be entered via hard copy form.  

PAYMENT MUST BE SUBMITTED FOR ENTRIES TO BE PROCESSED.  Please mail a check for $150 per entry and made payable to KAA Sweepstakes to the following address:

Look for the Sweepstakes Entry PayPal (you may use a credit card if you do not have PayPal) button on the home page.  Online payment option should be available by close of day on 12/17/21.  

Entries will not be processed until payment is remitted.
