Dual Credit Book Request
Public Academy for Performing Arts will be sponsoring the student listed below and we agree to pay the itemized costs associated with the dual enrollment course listed below, as long as the student passes the approved course.  This student is approved for required (not optional) textbooks for the course listed below.

Your child has requested a dual credit textbook.  By providing parent/guardian name/signature below, you are agreeing to the financial responsibility of each textbook need for your child's dual credit coursework as follows:
- All materials charged to PAPA are the property of PAPA and it is illegal to resell these materials to CNM or any other entity.
- At any time you no longer need these materials you MUST return them to the Public Academy for Performing Arts.  If a student drops or fails a dual enrollment course the student will be held financially responsible for the textbook.
- I understand that the books listed above must be returned to the Academic Transition Specialist at the end of the dual credit semester.  Further dual credit materials will not be permitted until the student returns the above listed textbook.
