A Diman Regional Work Order Request Form must be completed. Forms are available on the school website. A completed and signed Work Order Request Form must be submitted prior to the start of the work. Diman Regional's Automotive Collision, Repair, & Refinishing Program will assess the work required and, if appropriate, accept the work order. Once the request is reviewed, you will be contacted at the number or email you provided.
Submission of this request form does not guarantee acceptance of work request.
Once your form is submitted, it will be added to the list of requests. That list will then be used by the instructor to assist delivering curriculum. If your request does not fit into the current years curriculum, it will remain on a waitlist.
Due to the nature of the school setting and educational process, a completion date cannot be provided in advance. What would take a repair shop 3 days might take 3 months
Work will be accepted in accordance to the schedule and curriculum. Job requests will be evaluated by the instructor and assigned based on suitability for student skill development and availability of student scheduling.
Services provided are for educational purposes only, hereby releasing employees and students from any liability, loss or accidental damage while completing or as a result of completing a project.
Services provided are completed to the best of the students' abilities and may not be comparable to body shop standards.
The request must be relevant to shop curriculum.
The focus of providing services to the public is to enhance the learning process for students and should not be considered "professional services."
Any repair request for a vehicle that is more than "TEN MODEL YEARS OLD" may not be considered for repair.
If a vehicle is accepted for repair and upon further inspection it is deemed that the repair exceeds the scope of educational value the repair will be terminated and the vehicle will be released back to its owner.
All vehicles must be privately owned, not commercially licensed, and not a fleet vehicle. Vehicles or components will not be sold for profit.
All vehicles must be insured and registered (proof of registration may be requested).
Only vehicles from district customers will be considered. No vehicle from outside our sending district will be accepted. District customers are residents with vehicles that are registered in the sending communities of Fall River, Somerset, Swansea and Westport.
Any safety related work (brakes, steering, etc.) will be road tested by the students, who worked on the vehicle, and an instructor.
Work Authorization:
I authorize Diman Regional's Automotive Collision, Repair, & Refinishing Program to perform the repair work described on this order form, utilizing necessary labor, parts, and materials. I agree that Diman Regional's Automotive Collision, Repair, & Refinishing Program is not responsible for loss or damage to the vehicle, or articles left in the vehicle, in case of theft, fire, or any other cause beyond control. I agree that Diman Regional's Automotive Collision, Repair, & Refinishing Program is not responsible for delays during the course of the repair process. I grant Diman Regional's Automotive Collision, Repair, & Refinishing Program Instructors/Students permission to operate my vehicle for the purpose of testing and/or inspection. I understand if any closer analysis finds additional labor, parts, or materials are necessary to complete the repair, I will be contacted for authorization only if the amount of repairs that I will pay will be increased. Payment Policy:
*Parts and materials shall be paid for in full prior to the repair process by the customer.
*The instructor will provide the applicant with a pricing estimate before performing the work ( this estimate may change throughout the process.)
* If paint and materials are needed, the estimated value will go towards restocking necessary educational inventory (items ordered may differ from paint and materials needed for actual repair.)
*Credit/Debit cards are the only accepted form of payment.
*The applicant will be responsible to call vendor to process payment.
*NO WORK SHALL BE PERFORMED OR RELEASED UNTIL THE INVOICE IS PAID.
