WHITBY FC - Trial Registration 2021/2022 Season
Please complete and submit this form in order to welcome you to training sessions in October 2021 with our Grassroots Program (U9-U12) or our Competitive Program (U13-U17). Please submit the form once all field have been completed. If your training sessions are scheduled indoor (smaller Dome), the following regulations apply:

* All participants (players) are being asked to wear a mask / face covering when entering the dome (access to front desk). Masks / face coverings do not have to be worn during the on-field session.

* One parent / guardian per player will be allowed to observe the session from the spectator's area. All spectators need to wear a mask / face covering at all times !

* Anyone over the age of 18, has to provide identification and proof of full vaccination (indoor facilities only)
